I have:

Asus p8z77-v motherboard
Cpu: i5-3570k
Ram: corsair vengeance 8gb in dual channel
Graphics: Nvidia GTX 970
Drive: WD 1.5tb green

Have you tried Aptiofixdrv2? Be sure that you are not injecting. inject nvidia (false)

I have not yet tried Aptiofixdrv2 and I'm sure not injecting Nvidia. If you see everything is off

Your image shows "still waiting for root device"... It indicates OS X cannot mount root. When booting from USB, root is on USB. Hence, this is a USB problem.

Try different USB ports, or checkout the guide...

thanks for the answer, got the same problem and solved by using usb 2.0 port
