Your problem isn't graphics related.

Somehow with the same configuration I managed to installed 10.11.1
But when I restart the whole time the computer can't boot. I have to physically change the output of my two monitors with different configurations every time to get the hackintosh to boot. Meaning sometimes connecting one monitor to HDMI input from the Mobo and the other one connected through Display Port to the Graphics Card. Other times just having one monitor connected to Display Port and the other one disconnected.

So I don't understand why this problem isn't GPU related. If not then what could be causing this?

Add OsxAptioFixDrv-64.efi to the driver64UEFI folder in your Clover install.

This is my first build and hoping I can get this working properly...

Specs:
Gigabyte Z97X Gaming 5 Mobo
4790k i7
32 GB DDR3 Crucial Ballistix Tactical
Gigabyte Geforce GTX 960 4GB

Was having a lot of issues at first, but did a clean install and am now only having the monitor sleep upon boot up. I can boot with nv_disable=1. Made sure the profile was set to iMac14,2 and have the drivers installed, but still having boot issues. I have also dug through the posts in this thread and have tried the fixes I have found for similar issues with similar GPUs, although they have either done nothing or I can't quite figure out how to apply.

Any help is greatly appreciated!

what port are you using on the card?

Didn't even consider the port part of the issue.
Had it in HDMI, tried DVI, worked perfectly. I feel so foolish, but thank you so much!!

If HDMI isn't working then you might have Inject/Nvidia/True in your config if using Clover or you have the boot flag GraphicsEnabler=Yes if using Chimera or Chameleon.

I am new here. i upgraded my yosemite to 10.10.5 (build 14F1021) and then according to your post i downloaded and updated the driver to 346.02.03f02.. now whenever i start my mac, after boot screen monitor goes to sleep and then mac doesnt boot up.. mine mac only does boot in safe mode.. i didnt take backup using time machine. now i cant uninstall driver nor the update. please help me to get the drivers working. any idea how to fix this.?

Are you sure it doesn't finish booting?
