Why? Your 750 TI could be working today.

I tried DVI2HDMI as you suggested, here are my results:

On the ASUS GTX 750 Ti 2GB DDR5 (PH Series) I have,
1 x HDMI port
2 x DVI-D Dual Link ports
1 x VGA or D-SUB port

On the Dell S2440L monitor I have,
1 x HDMI port
1 x VGA port

So, in order to test if I have output on the DVI ports, I couldn't use a DVI cable but had to use a DVI-D Dual Link to HDMI Adapter.

I bought a DVI-D Dual Link to HDMI Adapter and tested the DVI ports with the existing HDMI cable as well as 2 NEW HDMI cables.
I got NO HDMI SIGNAL with all the 3 HDMI cables and both the DVI ports in OS X Yosemite.
The same DVI-D Dual Link to HDMI Adapter, all 3 HDMI cables and both DVI ports on the video card worked perfectly fine with Windows 8.1.

I even tested the HDMI port on the video card with the 2 NEW HDMI cables (the original cable was already showing NO SIGNAL with the video card HDMI port), still NO HDMI SIGNAL with either of the cables.

It appears that I am not getting output on ANY port of my ASUS GTX 750 Ti 2GB DDR5 (PH Series). Help!!

Edit: Thanks to toleda for pointing out that OSX is not capable of natively displaying in 1440p though HDMI, I previously was under the impression that it simply was not working.

I stand corrected.
Conclusions:
There is one value of NVCAP for all Maxwell graphics card as noted in Post #1,
If the NVCAP is different, 2 framebuffer connectors are enabled instead of 4; different problem
If HDMI is not working, DVI is not working and vice versa.
Asus graphics card with BIOS 82.07.25... or 82.07.32... are not working
EVGA with 82.07.32... are working

Does the GTX 750 work for BIOS setup (no OS)?

Not Working: HDMI 1440p
OS X does not support HDMI/1440p with any graphics system at this time. DP is required for any resolution over 1080p. SwitchResX may work as an interim HDMI solution.

Yes, BIOS setup and Windows 8.1 work on all ports with my ASUS GTX 750 Ti.

Btw toleda, check the attached image. I was wondering if the discrepancy between my IOREG and the IOREG you posted, as illustrated in the image, had anything to do with my problem (esp. no output on DVI). Does my card seem to be installed correctly?
[PS - Nvidia Driver Manager recognizes it in the ECC tab as Nvidia GeForce GTX 750 Ti and so does System Report despite the IOREG. Although, verbose boot detects it's device ID as an unknown HD Audio Device].

I am awaiting a 2560x1440 monitor (dell u2715h) only to find my onboard hd4600 will not support it via hdmi when it arrives. looking at getting an evga 750 ti to get that full resolution via dp (that means display port, right?) how does an upgrade go for installing one of these and is it currently the best (and cheapest!) card to get for my needs of browsing and procrastinating?

I am about to pull my head out the sand of using onboard graphics and start using dedicated a gpu so apologise for the stupidness .
