Make sure your USB is large enough (8GB or 16GB), is MBR, and the CLOVER EFI partition is 200mb (no larger).

Problem is still there even with 200mb Clover EFI partition.

Here is the partition scheme:

16gb USB with
200mb Clover
7.23gb MACUSB
8.45gb Data

When I try creating Yosemite USB, it works flawless. Even Clover EFI partition is about 8gb.

Installation does start, but after few seconds it gives error:

What error?

I was looking for "diskutil list" output when running Yosemite, not the OS X installer.

Not sure if it is related or not, since I re-installed windows I always get 2 options to select from now. However only first one works.

This is in your BCD file.(EFI\Microsoft\Boot\BCD). You can edit it with BCDEDIT.EXE.

But I think even USB is successfully created, I'll still get error while installation "Partition map modification would make a Windows partition unbootable." because I am currently getting this error even while installing Yosemite from USB.

Repartition HDD so Windows is not present. No Windows partitions should be created when partitioning the disk.

There are many restrictions if you have an existing Windows install on the disk.

Too small.

In the guide it was suggested to create > 7gb. So I just created 7.1gb to make max usable space for other data. Yosemite installer is actually working good in it.
Anyway how much space you suggest for El Capitan?

I had success with 7.96GB.
 
Don't have any Windows partitions on the disk during the install.

You can keep an existing Windows install, but there are issues/workarounds.

So what should I do now? Should I remove all partitions and then install everything again?
I have a partition Data, Fat32. Should I remove that too?

Another problem, now I can't even boot to existing Yosemite OS. After boot loading indicator finishes, it shows only black screen. If I arrive at clover screen using Yosemite installer USB, then choose MAC partition it boots successfully. But then there is no sound and battery sign on top menu. Any suggestions in this regard? I tried rebuild cache in kext application.
