No, My HD is not failing. :p

Most likely it is an ACPI problem, but without complete details on your install/post-install process I can't be certain.

okay here is my full process/details
-
I installed Yosemtie 10.10 through USB(Vanilla Installation)
Installation went smooth. Had to apply MBR patch to dual boot Windows/OS X on the same disk(legacy bios)
After Installation, poor graphics, No touchpad, keyboard, audio, ethernet, bluetooth was working
so in post installation i installed chameleon with dsdtfree from multibeast. rebooted
extracted dsdt with maciasl resolved all the errors, pasted it into extra and rebooted.
then i resolved graphics problem with this guide - http://www.insanelymac.com/forum/topic/286092-guide-1st-generation-intel-hd-graphics-qeci/

then resolved keyboard and mouse with your voodoops2 kext
audio with applehda patch in dsdt and patching my own applehda.kext
brightness control with your dsdt patch and kext
battery indicator with your dsdt patch and kext.
bluetooth working with dell 365 module package installer.
ethernet rtl8100.kext
and what else?

MBR patch is not needed.

See guide: http://www.tonymacx86.com/multi-booting/96000-guide-dual-booting-mountain-lion-windows-8-a.html

In addition, use/discussion of MBR patch is against the rules: http://www.tonymacx86.com/faq.php

MBR patched installer is not vanilla.

After Installation, poor graphics, No touchpad, keyboard, audio, ethernet, bluetooth was working
so in post installation i installed chameleon with dsdtfree from multibeast.

Chameleon is not available in Multibeast.

rebooted
extracted dsdt with maciasl resolved all the errors, pasted it into extra and rebooted.

Patching only for errors makes no sense. DSDT patched only for errors is functionally equivalent to unpatched DSDT.

You didn't mention power management.

There is a better kext for Ethernet, see FAQ.

I wonder if USB BT causes issues. You didn't mention what you installed for USB WiFi.

You should try a subset (eliminate unnecessary drivers/etc) to see if sleep works without them.

For example, a basic install with patched DSDT (according to guide), power management, and working graphics QE/CI should sleep wake ok.

I had to use that because i cannot boot with guid partition with legacy bios.

sorry i mean to say chimera

Patching only for errors makes no sense. DSDT patched only for errors is functionally equivalent to unpatched DSDT.

I had lots of errors in the extracted dsdt so i had to resolve those errors.
and i applied patches like rtc patch, IRQ fix( for working audio), Smbus fix, GFX0 > IGPU .

then resolved keyboard and mouse with your voodoops2 kext
audio with applehda patch in dsdt and patching my own applehda.kext
brightness control with your dsdt patch and kext

Powermangement is native. AppleIntelPowerManagement kext is used
I generate C states and P states in plist.

USB wifi with RTL8188EU.kext from realtek. i removed it yesterday.
Bluetooth switch is off.

i will definitely look at the ethernet kext once i get sleep working.

also i have usbbusfix=yes in plist

Still not needed. Install GPT, convert to MBR with gdisk after install.

When I asked for all details on post install, I meant *all*. Do you have an exact list of the patches applied? Which OEM SSDTs are you dropping?

No such thing as AppleIntelPowerManagement.

You mean you installed patched AppleIntelCPUPowerManagement.kext. Did you patch it yourself or get it from Multibeast?

Not sure what you mean by "Bluetooth switch is off". Bluetooth hardware is installed or not?

Best to use appropriate drivers first. As stated before, your problem with sleep could be anything. The word 'anything' does not exclude Ethernet drivers.

battery patch from your guide
brightness patch from your guide.
gfx0 to igpu
EHCI 6 series usb patch
fix system_WAK1

OS Check Fix
RTC Fix
SMBus Fix
I couldn't remember any other patch

and i am not using any ssdt.

Some questions not answered... Are you dropping OEM SSDTs?

If you're not certain of the patches you used, do it again. Keep accurate notes.

Please provide complete details in your profile/signature
(Profile/Settings link in upper right corner of this site)

System: manufacturer/model + bootloader(Clover/Chameleon/Chimera)
CPU: detailed CPU model + motherboard chipset
Graphics: all graphics devices + laptop internal screen resolution

For example, typical Ivy laptop:
Mobo: HP ProBook 4540s (Clover)
CPU: i5-3320m/HM76
Graphics: HD4000, 1366x768

Use CPU-Z on Windows to find CPU (Core iX-xxx) and motherboard chipset (HMxx), and graphics capabilities. For a laptop, these details are important and affect critical installation procedures.

Mobo - Dell Inspiron 1564 HM55 (Chameleon)
Pro - Intel Core i3 2.13GHz 330M,
Graphics - 1st Generation Intel HD Graphics
Ram 3GB
320GB Hard Disk

and I think SSDTs are only helpful in sandy bridge processors. its not a sandy bridge laptop
I told in the previous answer that i am not using any type of SSDT
and I am sure that i haven't used any other patch in my dsdt.

Let me ask the question more directly... since you don't know what it means to drop OEM SSDTs...

Are you using DropSSDT=Yes?
