yes I'm using ig-platform-id 0x19120000,
and just used PlistEdit Pro, seems that the patch is ok, but still no 3rd display from that HDMI 2.0 port, maybe the issue is that it is HDMI 2.0 and not HDMI 1.4

I don't think so, may be port is enabled but signal out of range, connect only HDMI 2.0 port at boot up that can verify whether FB@ patch work or not.

If system caches are updated correctly, it should work, look at previously attached png, my LG Ultrawide need to downscale form 21:9 to 16:9, otherwise 3 displays output are impossible for me, can you upload ioreg file with 3 monitors connected, may helpful for troubleshooting.
 
I tried to boot up with HDMI 2.0 only, with a normal 16:9 screen, but the screen is losing signal when reaching 3/4 of the loading bar in the screen with the Apple logo.

So it seems the patch is not working.

I was following that guide when I built my mackintosh:

And there was that message in the guide, that's why I'm afraid that port will never work:

What Doesn't Work At All
  • HD movie previews in iTunes, suggesting that any rented or purchased movies may not work.
  • If you were to use integrated graphics, the HDMI 2.0 port does not work (tested on a Samsung 4K monitor with HDMI 2.0 support). The HDMI 1.4 port works fine, but at 30 Hz for 4K.
  • Sleep/wake -- seems to crash on wake, but may yet be correctable
Anyway, I attached the ioreg file with the 3 screens connected (even if the 3rd has no signal)

I can not load your ioreg file, use attached file to generate again.

It's because I used the v3.03, here's the ioreg file with the v2.1

Patch is work, sequence order has rearrange, I am afraid that your HDMI 2.0 port may linked to FB@0, if so, this patch will not work for you, 0x19120000 id FB data FB@0 is disabled.

Need check others ID or patch, may be can find a way to enable it.

Btw the way sysdef iMac 17.1 have multi displays problem with me, I use iMac 14.1.
 
It's because I used the v3.03, here's the ioreg file with the v2.1

I make a FB@ patch with ig-platform-d 0x19160000, checked with 10.11.6 and 10.12.5, ioreg show FB@0 seems to be work, connector type has change with patch, I do not know whether it can help or not, maybe you can test it, the best is also change sysdef to iMac 14.1, backup your config.plist first.

For 0x19160000 test only.

I just tried, but same results, and only the DP port is working now.

ioreg attached with iMac 14,1 and the ig-platform-d 0x19160000 patch if you want to check.

Result is normal, FB@ 0306 port 7 is not existed in 0x19160000, patch only use for check whether HDMI 2.0 is linked to FB@ 0000 port 0 or linked to FB@ 0204 port 6.

Ioreg show that connectors type remap as expect, correct ig-platform-id should be 0x19120000 not 0x19160000.

Currently, seems to be no way make HDMI 2.0 work.
