BAM! With trying other default frame buffer personalities, I got VGA working! I try to combine it with my customised personality and I got dual monitor, laptop + VGA working at the same time. The only issue left is that, the resolution of VGA output detection is incorrect. It does give really high resolutions but none of them seems to come from EDID of the monitor... I will keep posted of my progress.
Update #1:
No matter how, I cannot get resolution working on VGA monitor. I thus check its official website (ThinkVision LT2452p) and it doesn't say that it supports Mac system. So I assume this is the problem and I will not go further experimenting with it. However, none of the Hot Plugs working, neither the laptop screen nor the VGA monitor. Changing hot-plug-id doesn't help as well. I will leave it for now and enjoy VGA working first.
Update #2:
There is no way that the OS X is picking up EDID from my VGA monitor. Therefore I have followed this post to provide the EDID it needs. Now the resolution is fully working!
I have also tried to use SwitchResX, but somehow it doesn't work at all. Could be that I am running El Capitan now.
