Unfortunately, I don't have any further advice. I documented it as thoroughly as I could above. Try starting from scratch (reset the BIOS to its factory defaults, then make all the changes recommended). Make sure you're using a USB2 flash drive in a USB2 port (not USB3). Update: a different USB drive fixed it (I believe). The old USB drive was a USB3 drive, while the new one that worked was a USB2 drive (I believe). Both were plugged into USB2 ports at all times, though.
