Snow artifacts in QuickTime icon preview

Running 10.11.3 and every QuickTime on my desktop has snow for it's icon preview.
The videos play fine in QuickTime. Sometimes they show up snowy in messages app or Mail app.
They DO NOT play correctly in QuickLook (see video here http://www.kalukalafilms.com/filechute/video in Quicklook.mp4).
I'm using a GTX 680 with the 346.03.05f01 nvidia drivers. The intel gpu driver is disabled via BIOS. Inject Intel is enabled in Clover (v3330). Can't figure out what is causing this problem and couldn't find a fix in the forums. Anyone got any clues?

Unfortunately it is a bug that we have to deal with. You can try disabling the IGP in your bios (if it is still enabled) and sometimes that helps.

Nope. No solution yet for me.

Same issue happening to me when I open youtube videos or system preferences magic mouse settings gif video also mp4. mov. thumbnails. This issuue just on thumbnails not on the playing video.

I turned my primary graphics to Intel HD and all these artefacts gone.
