This time, I brought a different disk. I clean the disk and clearly installed 10.9.4 OSX in it.
after install the "Azul-10.9.4-9series.zip" by using kext utility, here are my achieves you requested:

10.9.4 version of the kext in top post updated with proper fix. Apologies for any confusion.

I clean the disk and clearly installed 10.9.4 OSX in it.

The problem was not your install. Fix:
1. Delete System/Library/Extension/AppleIntelFramebufferAzul.kext
2. Download 10.9.4 Azul..., Post #1
3. Install with Kext Utility
4. Restart

Reply with new IOReg.
