OpenCore EFI updated to version 0.6.5.

As usual, do not use your old config.plist. Instead, copy/paste your Serial, Board Serial, UUID, and MAC address to the new config.plist.

Did you check the metal screw behind the BCM94360CS2 yet?
That was the issue, removing the screw fixed it immediately. Is that screw-hole a contact on the card? Seems like an odd design choice...

My adapter didn't come with any plastic screws and even the riser is metal, would it be fine to just leave it without a screw? It's fairly secure inside the adapter as is.

Mine came with a plastic screw... You can just put a piece of tape over the screw to insulate it from making contact with the BCM94360CS2.
 
I used your oc efi.I did the usb map,but stuck here.It's my efi.How to fix it
