As usual, do not use your old config.plist. Instead, copy/paste your Serial, Board Serial, UUID, and MAC address to the new config.plist.

That was the issue, removing the screw fixed it immediately. Is that screw-hole a contact on the card? Seems like an odd design choice...Did you check the metal screw behind the BCM94360CS2 yet?

My adapter didn't come with any plastic screws and even the riser is metal, would it be fine to just leave it without a screw? It's fairly secure inside the adapter as is.
