Yes, ideally, you want to see all 26 ports so that you can map them out. But, I think we can try and cheat a little...

If you look at my USB ports, you can see that the Super Speed (aka USB3) port numbers correspond identically to the High Speed (aka USB2) ports. It was exactly the same situation when I mapped out the ports on my previous build. Therefore, I think we can cheat and just assume that the SS port numbers will be the same as the HS port numbers.

This should allow you to make your SSDT.

I followed your method, and mapped my USB ports successfully! It's very helpful, thanks a lot!

I also found that "uia_exclude" works without the SSDT file, so I can disable ports I can see, and reveal missing USB3 ports. With that, I can confirm USB3 port numbers are correspond identically to the USB2 ports on my board too.

Here it's the USB mappings I use for my ASUS Rog Strix Z370-i motherboard:

I planned to use 2 samsung 960 evo ssd for my system. One for window and another for Mac. Is these any problems I should know before order? Thanks a lot!
 
Maybe I did something wrong, but on my Z370M-iTX/AC the USB2 port and USB3 port numbers do not coincide

Maybe ASRock does it differently. I don't know. I've never had an ASRock motherboard to work with. I've only done it for two Asus motherboards and now WuTian has confirmed that his Asus motherboard is the same.

Yes. uia_exclude works without an SSDT. But having an SSDT is still important because it helps identify the internal ports. This is particularly important for Bluetooth and helps with sleep/wake.

Yes, the 960 EVO works fine. I used it on a build for a friend. Just avoid the Samsung PM981. The PM981 causes random, violent reboots and no fix has been found yet.
 
BIOS 0615 has been released. Everything seems to be running fine with it.

Which ports are you having problems with? What is your uia_exclude= setting?
 
I have a problem only with usb, 4 ports 3.0 on backside, another ports like 3.1 100% speed.HS12 its my bluetooth dongle

I think you need to add one more port to your uia_exclude=. You are still over 15 ports.
