Well, nothing unexpected in _WAK. Do your brightness keys work to bring it back?

Is suspect something wrong happening in VHIV (called from VWAK, called from HWAK, called from _WAK).

Does it happen if you disable the call to _OFF?? change it to: "if (0) { _OFF() }"

Not sure what you're saying there...

You have to redo the patch, but don't apply the part that adds the _OFF() call.
 

You have a problem with your SSDT setup. You need to drop all OEM tables, then include patched ones as appropriate. OEM tables will interfere with patched tables.

In addition, I did not find a patched SSDT loaded in your ioreg, so you have issues with copying files to your /EFI/CLOVER/ACPI/patched directory.

If you check your Clover bootlog, you'll discover you have only ssdt.aml in ACPI/patched.

Btw. without the _OFF patch the card is still recognized.
