Ioreg shows AppleDisplay on Port 0. Port 1 shows no AppleDisplay. Are you using VGA? Or HDMI, DVI combination?

hdmi and dvi ! only hdmi works but if i plug off the dvi alsi the hdmi stops!!
if hdmi and dvi are connected works only the hdmi

Which 5450 Variant do you have? Sapphire?

No ideas. When I tried 2 monitors on the 5450 I had it worked. Maybe try using SSDT injection instead of Clovers FakeID. Or use onboard iGPU instead.
