Attach EFI/Clover folder as ZIP (omit 'themes' folder).

Now it shows "busy timeout(0), (60s) kextd wait (0); "SAMSUNG MZHPU512HCGL-0004 Media""

Yes, of course.

I have tried several times rebooting and still "busy timeout(0), (60s) kextd wait (0); "SAMSUNG MZHPU512HCGL-0004 Media"" appears. However, I got 1 single time out of maybe 10's that typing "diskutil list" in terminal in fact shows me my SSD and it disappear again after reboot.

And when I open the system report page, it actually recognise the SSD but it just don't show in the Disk utility or finder.
Occasionally after reboot, the disk utility will detect the SSD but it is not accessible as it will keep loading when I open Disk Utility GUI.

FixRegions_10000000 in your config.plist.. not a good idea. Remove it or set false.
Your SSD is PCIe/AHCI... it should work with the default drivers, but may have a compatibility problem. Do you have the latest firmware on it?

Other settings in your config.plist are questionable...
See guide for better starter plists:

Your SSD may have compatibility issues. Is the firmware up-to-date?

The SSD has no firmware to update as it is a client SSD and I actually boot it with 10.11 successfully before. That's why I am so confused of why it is not working.

This is what it shows when it recognised. The disk utility keep loading and after that, it just unmount the disk.
I guess there is no another solution than buying another SSD?

I managed to get my XP941 useable now. Since I can boot 10.11.3 before, I extract some kext from 10.11.3 to 10.12.3 and it now can shows the use and install to the SSD. I replaced AppleIntelLpssI2C.kext and AppleIntelLpssI2CController.kext with the 10.11.3 version and it works now. I really don't understand why. LOL
