Dell Precision 5510 (Almost same as xps 15 9550) Corrupts disk

After a few restarts my disk gets corrupted which is very strange because that's a brand new laptop. I ran dell's diagnostics too but shows no problem. I reinstalled the system twice now but it keeps failing.
I noticed though that when it worked, it had problem installing the java sdk. And after a restart I got this screen that I attached.

I attached the screen I get when I try to boot up. Attached my clover efi files too.

Before hackintosh an ubuntu 15 was on it for a week. It didn't have any problem.

Do you have any idea what could cause this problem?

Are you using software (Paragon) to access HFS+ drives from Windows?
 
There's no other operating system installed.

Mobo: manufacturer/model + bootloader(Clover/Chameleon/Chimera)
CPU: detailed CPU model + motherboard chipset
Graphics: all graphics devices + laptop internal screen resolution

For example, typical Ivy laptop:
Mobo: HP ProBook 4540s (Clover)
CPU: i5-3320m/HM76
Graphics: HD4000, 1366x768

Use CPU-Z on Windows to find CPU (Core iX-xxx) and motherboard chipset (HMxx), and graphics capabilities. For a laptop, these details are important and affect critical installation procedures.

Note: When specifying your bootloader, if using Clover legacy, please be sure to specify "Clover legacy"). Specifying "Clover" will be assumed as "Clover UEFI"
 
It's a Toshiba thnsn5256gpu7 nvme ssd 256gb.

Attach ioreg as ZIP: http://www.tonymacx86.com/audio/58368-guide-how-make-copy-ioreg.html. Please, use the IORegistryExplorer v2.1 attached to the post! DO NOT reply with an ioreg from any other version of IORegistryExplorer.app.

Attach EFI/Clover folder as ZIP (press F4 at main Clover screen before collecting). Please eliminate 'themes' directory. Provide only EFI/Clover, not the entire EFI folder.

sudo touch /System/Library/Extensions && sudo kextcache -u /

Compress all files as ZIP. Do not use external links. Attach all files using site attachments only.
 
I can only get into recovery. Can't start the system with usb either.

Repair with FirstAid in Disk Utility from within the OS X installer.
Your problem may be incompatibility of the NVMe SSD with Pike's patches.
