You probably need to re-download the package from the Mac App Store then. Just a guess...

Then re-apply installed kexts before the system restarts.

Did you have to delete any kexts when you had it working before?

IOBluetoothController.kext when booting is graphics related so it is probably the nvidia optimus card playing up. Either disable it in the BIOS or delete nvidia kexts inside of /System/Library/Extensions, try booting in safe mode to do this (-x).
