HELP Issue with Graphics Card

See attached... The "Verbose Nvidia" picture is what I just got when I set my Nvidia card as default display, disabled IGFX and physically installed it and tried to boot. This error is different than what I was getting before. Before, it ran through the whole boot and at the end when it would load the Nvidia drivers, it would crash. An example of this is in the "Verbose Old" picture, which I just found actually, I forgot I took that.

I also get this issue sometimes when I first boot up even on the IGFX, but after a reboot or 2 it goes away, see "Verbose IGFX" picture for this, it has error loading kernel cache in there too

Edit: Ok I don't think you can see the file names, if not, the one with the date on it is what I referred to as "Old", the error loading kernel cache is "IGFX" and the other is "Nvidia"

I'm using a DVI cable .When I am trying to boot with my Nvidia card in, I'm on port 1, I tried port 2 as well. Same issue. When I enable and use my IGFX, I'm plugged right into the motherboard port.
 
Choose one and stick with it - If you keep changing it will be more difficult to diagnose.
You should probably zip and upload your config.plist.

I want to use my Nvidia card and have IGFX disabled, I am only switching so I can use my computer, but yeah fixing my Nvidia card is what I'm focused on. config.plist is attached

Boot with the attached config.plist and show picture of the screen when it stops.

Ok, doing it now. Also, should I have FakeSMC.kext in my EFI partition in my kexts/10.11 folder? I just checked and that folder is empty, will report back soon on the config.plist

No, I choose to put my kexts in /kexts/Other and delete all of the 10.* folders.
