I replaced it, rebooted and the usb 3.0's are still not working. any other suggestions?

Hmm... The proper USB 15 port limit patch is applied. USB3 should be working. Do you know where USBInjectAll.kext is installed? It should be either /EFI/CLOVER/kexts/Other/ or /Library/Extensions/. Try replacing it with the latest version from:
https://bitbucket.org/RehabMan/os-x-usb-inject-all/downloads/

Installed in into efi/clover/kexts/other and nothing improved after reboot. Not sure where the /Library/extensions is you are referring to.

I installed it in both locations now and neither worked after rebooting the system. I am at a loss at this point.

It shouldn't be in both locations. Only one. Where was it originally installed? Did you use a kext installer to install it in /Library/Extensions/? If not, you will have to rebuild the kext cache.
