1. I upgraded to 10.8.5 from 10.8.2. Sound worked perfectly for 4 days. This morning, all sound (input and output) failed. Sound input Should be from my Logitech C920 cam and output should be some attached speakers that are connected to my Dell monitor. According to Sys/Prefs/Sound/input, the logitech microphone is working, but while using Skype, the other party could not hear me (his sound was working). According to Sys/Prefs/Sound/output, my external and internal speakers do not show up. Only a LogMeIn sound driver shows up.

4. I don't believe that there are any others.

Version of S/L/E/AppleHDA.kext?

I had it listed at bullet point #3: 2.4.7-toledaALC898

(Personally I thought it was pretty cool that it has your name on it ;) )
