It is, at best, difficult to debug a problem remotely when you're entirely relying on someone else to convey the actions, issues, and observations via written text.

It is somewhat more difficult when the person who is the go-between insists that the diagnosis they are receiving must not be correct.

So. You have received our advice. You are welcome to troubleshoot the failure of the Bluetooth functionality on your combo card in any manner you like. May the Force be with you.

If that were the case, does it make sense that the USB port works for the mouse and for the Bluetooth dongle, but not for the combo card?

It's strange, but what else could be the cause? The whole USB stack has been reworked in 10.11, so it surely has something to do with that.

Start out by installing FakePCIID_XHCIMux.kext, and see if that fixes it. If not, you can then go through the whole El Capitan USB guide.
