HD 4000 4MB VRAM even the platform and fakeid is set to 0x01660003

The ioreg shows it was booted with a bogus FakeID (0x01660003). You should not expect the graphics drivers to load when using an invalid FakeID. HD4000 does not require FakeID.

Connect external monitor. It is likely an EDID problem (you can try config.plist/Graphics/InjectEDID=true).

i already try inject intel and inject edid in clover setting..
but still

You clearly are not using the correct config.plist. That config.plist is for Skylake or Broadwell (minStolenSize patch).
