For example, typical Ivy laptop:

CPU: i5-3320m/HM76
Graphics: HD4000, 1366x768

Use CPU-Z on Windows to find CPU (Core iX-xxx) and motherboard chipset (HMxx). For a laptop, these details are important and affect critical installation procedures.

Remove the nvidia kexts (NV*, Geforce*).

Then boot with:
IGPEnabler=Y GraphicsEnabler=N IGPlatformID=01660003 -v dart=0

Usually that is an indication you saved your DSDT in the wrong format. You cannot place a text file at /Extra/dsdt.aml and expect it to work. Verify you saved in AML format (ACPI Machine Language Binary).

Looks like you have some duplicate kexts. If you're going to use my driver, make sure you follow the installation instructions.

And you probably have a graphics issue now...

Are you using Chimera bootloader?

If so, IGPEnabler=Y GraphicsEnabler=N IGPlatformID=01660003 -v dart=0

Since you're using Chameleon, you need different flags:
GraphicsEnabler=Y IntelCapriFB=3 dart=0
