I resolved it by following this post, should have done this before https://www.tonymacx86.com/threads/macos-10-12-5-update.220866/#post-1504116 restoring the image to 10.12.1 and then installing the kext and updating to 10.12.5

I have purchased another card BCM94630CD combo card and it worked straight out of the box. I signed out from iCloud, iMessages, FaceTime and Appstore and signed in back again which enabled continuity.
