BCM94360CS instead of CD?

There's decent likelihood that it would still be reported as an Apple product if it were an Apple prototype.

BT is working over USB only 3.3v it gets from from PCIe. Check bottom backside of wifi card for missing elements. It is not so rare problem that resistor is missing. It could be detected as snapped of solder points. Use magnifying glass.
 
Your's looks identical to mine. Contact the seller and tell them that they sent you a defective one.

Those are Macbook Air WIFI cards. Totally different to iMac WIFI/Bluetooth combo cards. You're card could be defective.

Those are Macbook Pro Retina cards actually. Violet dragon could you please test yours for BT RSSI in few scenarios like 1, 2, 5 meters of BT device from antennas?

Here is mine on Magic Mouse one meter from antennas 1) with WIFi On 2)WiFi Off 3) WiFi Off and Router Off

Macbook Air has same WIFI/Bluetooth Combo cards from what I've seen anyway as i repair the things. I don't have any problems with Bluetooth on my desktop. Ive used some Laptop antennas and stuck them under the desk.
