Weirdly enough, I solved Issue #2 by replacing the Youbo card with an MQUPIN Fenvi T919 card. For me the MQUPIN card has been more stable - sleep wake, watch unlock, etc. Just posting this in case it helps anyone.
@CaseySJ Is it recommended to sign out of iCloud & iMessage when swapping out the WiFi/BT card? My card (Youbo) has been giving me some weird issues so I want to try my Fenvi T919 to see if it helps.

(Have the same ASRock ITX Motherboard on the way for an AMD hack so trying to see if I can do it without using an SPI reader)
Just in case anyone's curious, I have the following workarounds in place to tackle the above issues:
Allow Bluetooth devices to wake this computer => I had to keep this enabled to ensure I could turn BT ON and OFF after sleep wake cycles
I ended up disabling "SSDT-USBW.aml" and...
@CaseySJ After a few users posted their TB3 IOReg tree, I checked mine and it wasn't configured correctly. I tried the V2 one and didn't have any luck.
What worked for me was the one posted on the HackinDROM website for Z490 Vision D. It has your name on it so I'm assuming you created that one...
Yes, even with this SSDT, System Report > Thunderbolt still says "No drivers". It shows up in the PCI section. However, sleep does work fwiw => macOS Big Sur 11.0.1

I went to the HackinDROM website and selected ASRock Z370/390. Replaced RP05 w/RP21 for our ASRock Z390 board and, well, here are the results:

@CaseySJ I'm experiencing the following issues with my Youbo WiFi/BT card:
After a few sleep & wakes and especially after long ones, I cannot toggle BT anymore. BT still works fine, but if I turn it OFF, I cannot turn it back ON.
Apple Watch Unlock doesn't seem to work after sleep & wake...
@mango1122 and for anyone else trying to get TB3 to work reliably.
I saw this post from @CaseySJ here and figured I'd give that SSDT a shot. Switched back to the original (NVM20) firmware using tbpatch and to my surprise, it has been working rather great. I'm testing with an OWC 12-port TB3...
