I updated my profile a bit but it was mostly correct, I'm on a desktop with HD3000 graphics from a i5-2500k, plus TL-WDN4800 and GTX580. Previously also had a USB3 card attached, but removed it and it didn't help with the reallocation error.
Ah, that's a shame - it doesn't seem to work on my board (reallocation error - same as this user). I've read that it can fail when having >2 PCI-E devices attached, which I do.
Oh, I see. I set the memory to preallocation to 64M and DVMT to 'Max' but that made the artifacting much worse - moving grey squares and lines all over the screen. With DVMT Prealloc=512M and Max DVMT='Max' I normally I only get thin white lines here and there.
I have slide=0 set that helped tremendously to reduce (but didn't eliminate entirely) the artifacting. I'll see if a new revision of Clover (UEFI) addressed my boot issues.

For what it's worth, this may be related to 10.11.4. My desktop (i5-2500k) HD3000 has been working fine up until I installed 10.11.4, and nothing I tried would get rid of the artifacts. I restored my 10.11.3 kexts and things are working normally again.
Same issue here on a Z68A-D3H-B3 after using MultiBeast. I suspect it has to do with kernel caches, as booting with the '-v' flag (just type that in when you're selecting your installation in the bootloader) works fine.
Are you using Chrome to play music with USB audio output by any chance? If so, there's a known bug in Chrome about this. Selecting a different audio output from System Preferences and then choosing your normal output again will work around the problem until it gets fixed upstream in Chrome.

Just happened to me twice in two days, but the machine often goes for 2-3 weeks at a time with no freezes.
How are you testing for full acceleration? As well, be sure that you ran the cache regeneration steps... I've found that you can get some bizarre results if you don't.
