I, too, suspected Boot Mode to be the problem. However, I tried setting
Intel Virtualization Technology = Enabled
VT-d = Enabled
Boot Mode Selection = UEFI and Legacy
but it didn't help. I'm at a loss for how the remaining three options could cause the keyboard and mouse to stop working, but...
When installing Windows I went from my working osX config to the default settings provided by the BIOS. When first installing osX I went from the default settings to the ones in the OP, or more detailed:

Thanks for the tip! Set my PCIRootUID to 0 and suddenly my graphics card popped up on the hardware list even though BIOS was detecting it with =1 all along :P I also dropped the npci=0x3000 as it wasn't doing anything.

I had the exact same problem as OP except my USB did not have legacy support on it. Booting with only "-x -v nv_disable=1" solved the problem for me. Thanks alot :D
