The following file can be downloaded from somewhere? or how do I can get?

AppleUSBXCHI.kext

What? AppleUSBXHCI.kext is provided by the OS X installer. Read the guide in post #1, follow exactly.
 
I just wanna say Thank you to Rehabman, I followed the guide and everything works fine on my hp4540s, but i miss one step with smbios(serial number) and wanna ask I missed important step ? or it's fine if everything works fine.

It is an important step. Follow the guide as written.

I've successfully installed OS X 10.11 using this guide. Thank you for making it. Much appreciated.

However I have an issue. I am using an external screen (HDMI) and I can't adjust the volume if I am using the external screen as an audio output device. On OS X 10.9.5 it was working fine. Is it default or something is not working correctly? Everything else is working fine.

It is per Apple design.

No volume controls for HDMI devices. It can be done with 3rd party (hacky) apps.

Volume control is on the device itself.
 
I ve just got a security update. How can I install a system update without any issues? Which steps should I follow?

If everything in EFI/Clover is up-to-date you should be able to update without issue.

So I did it and I could not boot. I've followed the steps from "Major updates". It's back to normal buy when I plug in my external display the system crashes.

Your post is not clear.

If you can boot...
Read post #1, "Problem Reporting".

If you can't boot...
Boot verbose, show photo. Attach EFI/Clover folder as ZIP (omit 'themes' folder).
