Weird USB Header issues

In an attempt to get my bluetooth working in Sierra, I've somewhat run a ground on what to try next or even if I'm going in the right direction. I have a bcm94360cd card with the adaptor in my PCI 1x port along with the card plugged into a usb 2.0 header on the motherboard. No bluetooth shows up in system info but the wifi works just slow. No PCI data shows up in system info either. I do not see any of my usb 2.0 ports showing up under usb and all of the integrated ports on the motherboard seem to work along with a usb 3.0 header I have plugged in for front ports. The xhc doesn't show my usb plugged in the front usb 3.0 ports but it works. I'm thinking the reason my bluetooth doesn't work has something to do with my ports not being configured. I haven't found much info on using an Asus z170m-d3h for configuring however. I hope someone can point me in the right direction. Thanks!

USB configuration isn't correct. What about SSP ports? Only have HS ports. You need to redo SSDT-UIAC. Have you inject AAPL Vales for XHC?
 
Okay I opened up the port limit from 15-26 but I'm not too sure how to assign them given that no devices are showing up in IO Registry Explorer. Have tried both usb 2.0 and 3.0 flash drivers. Any ideas? Thanks

So I found my ports that I was looking for, however maybe I'm missing something but why wouldn't my bcm94360cd show up in any of these? Should I try connecting a usb pci card to one of my mobo headers?

Red means the device has been disconnected. Better to use a USB 3 hub as it mounts as both USB 3.0 & USB 2.0. Note Bluetooth is internal needs to be under port 225. Attach ioreg.
