Triple Monitor Problem HD4000 / ATI5450

Until recently I have used two Dell U2713HM 2560x1440 monitors without a problem. One is connected to the HD4000 DisplayPort output and the other is connected to the ATI5450 DVI output.

I want to add a third monitor and I had an old HP LP2065 1600x1200 monitor to test. I connected this to the HD4000 DVI output and everything worked without changing any settings, both in OSX and Windows.
I used the HP in landscape and the two Dells in portrait.
Since the HP's image quality is not on par with the Dell's I swapped the HP out for a Dell U2412M 1920x1200 monitor and then the problems started.

With the same settings and connections as the HP I cannot get the Dell U2412M to display a desktop. It shows the BIOS screen when booting but nothing after that. It only works when I connect it to the Displayport output but this is not an option because the DVI output does not support 2560x1440 for the Dell U2713HM.

Even stranger is that when I connect the Dell U2412M as the only monitor to the HD4000 DVI output and boot up into Windows (not OSX) it also does not display the desktop?
I've tried different BIOS settings for the HD4000 memory size but this does not solve the problem.
If I connect the Dell U2713HM to the HD4000 DVI output it all works, but not at the Dell's native 2560x1440 resolution.

The only way I got the Dell U2412M working was when I set the ATI5450 as the primary VGA adapter in the BIOS (in lieu of the HD4000) and booted into Windows so that tells me the DVI input of the Dell U2412M does work. Unfortunately these settings do not work when booting into OSX, I get an FC when booting.

I am going to test another 24" monitor tonight but am I forgetting something? I just don't understand why the HP 1600x1200 works without problems but the Dell 1920x1200 doesn't work?
