After the cold boot, it looks like this:

How about a cold boot with the monitor unplugged? Does the full controller load then?

Because @nottooshabby used the "search" feature in IORegistryExplorer, the rest of the Thunderbolt device tree is hidden. This is why we don't see DSB2 with the USB hub. This is also why I discourage people from using the 'search' function in that app. But if we use search, we should then click little 'x' to the right of search bar to show the full tree.

Do you have the USB 2.0 cable from the AIC connected to the header on the Motherboard? I found when I connected that, my studio monitor stoped having issues on cold boot and after sleep.
Yes, it is connected and that port is mapped.
 
My bet is you’ve got the pin orientation upside down. Let us know what you find.
