System with dual monitors hangs on boot after 10.8.5 upgrade

I upgraded to 10.8.5 last week-end and after fixing the boot 0:error problem was able to boot from disk. Today the network card quit working so I tried a reboot. Instead of seeing the tonymac boot loader screen I get a text menu:

I remembered I didn't have the second monitor plugged into the HDMI port when I upgraded. Disconnected the HDMI and it boots up with the TonyMac logo screen.

Tried turning off the monitor with it plugged in to HDMI -- same problem. Switching monitor to digital port also does not work.

Any idea how to be able to boot with both monitors plugged in?
 
Got the same problem in mavericks, when i plug scnd monitor to hd4000 i got black chimera without gui....

any help?..
