stuck at prohibited sign (i5 6400 - gigabyte G1 sniper B7)

Hi.
i'm stuck at grabled message and prohibited sign (see below photos)
i already change my installation to any usb2 port and using cpus=1 too

Hardware :
i5 6400 2.7 Ghz
Gigabyte G1 Sniper B7
kingston 16GB
2TB WD HDD
Gigabyte Geforce GT 740

  To access BIOS/UEFI Setup, press and hold Delete on a USB Keyboard while the system is booting up

• Load Optimized Defaults

• If your CPU supports VT-d, disable it

• If your system has CFG-Lock, disable it

• If your system has Secure Boot Mode, disable it

• Set OS Type to Other OS

• If your system has IO SerialPort, disable it

• Set XHCI Handoff to Enabled

• If you have a 6 series or x58 system with AWARD BIOS, disable USB 3.0

• Save and exit.

USB related issues.

Check USB related BIOS settings.

Also try with USB 2.0/3.0 devices in both kind of port (USB 2.0 & USB 3.0).
