That looks normal to me. Is this a fresh install? No patches to BT kexts?

Well this problem just started few days back. It was working perfectly fine and I dont remember making any changes. And I have not patched any Apple kexts.

Do a fresh install.

By Fresh Install you mean Install over the current installation or wipe and Install ?

Yes.

You said it suddenly started happening without changes. Which usually means it started happening but the changes made were overlooked/forgotten.

Even after Clean Install I have the same issue. Attaching ioreg before and after sleep

This is not a very "clean" install. Still see Android File transfer, Little Snitch, Intel Power Gadget, VirtualBox.

Do a "clean" install. Do not install stuff that is not needed.

You can also try with FakePCIID_XHCIMux.

Before I do one more clean install I wanted to try using BrcmBluetoothInjector.kext. I installed

Code: 
BrcmBluetoothInjector.kext

BrcmFirmwareInjector2_13d3_3404.kext

FakePCIID_XHCIMux.kext

at /S/L/E

1. I am not getting "Bluetooth: Not Available" error after sleep.
2. Looks like the firmware is not getting loaded on startup and after sleep.

When I run

sudo touch /System/Library/Extensions/ && sudo kextcache -u /

I get the following messages

Wrong. Firmware will not be loaded using BrcmBluetoothInjector.

Read the README carefully.

When I run

Nothing wrong with the Info.plist that is checked into the repo. Must be incorrect edits by you...
