So I did a full reinstall of the machine. Steps as follow:

1. Create Unibeast installer with 10.12.5 on my MBP.
2. Boot using Unibeast into the installer.
3. Use disk utility to Erase the Mac SSD partition.
4. Install OS X to Mac SSD partition.
5. Install Clover_v2.4k_r4061 with EmuVariableUefi-64 option.
6. Reboot. Conclusion: Machine won't boot for some reason.
7. Boot using Unibeast into Mac SSD partition. Booting sequence works fine.
8. Install Multibeast (UEFI Boot Mode, Misc --> FakeSMC v6.25 (as it is mandatory), Clover v2.4k r4063 UEFI Boot Mode, NVIDIA Web Driver, System Definitions --> iMac 14,2).
9. Reboot. Booting sequence works fine now.
10. Install NVIDIA Webdriver.
11. Reboot. Come to the conclusion "Web driver" can't be selected.
12. Install Clover_v2.4k_r4061 with EmuVariableUefi-64 option again.
13. Reboot. Booting sequence works fine this time (i guess Multibeast inserted some kexts needed?)
14. Select Web driver again.
15. Reboot
16. No luck. still OS X Native driver selected.
17. Rerun Clover_v2.4k_r4061 with the "Install RC scripts on target volume" option.
18. Reboot
19. Select Web driver again.
Here is where it gets strange:
20a. When i boot using the "verbose" option. It boots fine, but has OS X Native driver selected again.
20b. When i boot without the "verbose" option. It boots into black screen.
21. Doing the method 1 of problem 4 in this post: https://www.tonymacx86.com/threads/solving-nvidia-driver-install-loading-problems.161256/#Problem4. Ran Kextbeast and installed the 2 kexts to /Library/Extensions.
22. Reboot without verbose mode. Still black screen. Using VNC i can not reach the machine (while when i boot with -v succesfully, i can use VNC to reach the machine).

I'm clueless what is happening here. I hope one of you guys has an idea.

EDIT: Added my EFI partition as attachment.
