SB Live! 5.1 (emu10k1)

Hi, I have installed ML on my hackintosh, but I can't get working SB Live! 5.1. I can see this card with lspci -nn

05:00.0 Multimedia audio controller [0401]: Creative Labs SB Live! EMU10k1 [1102:0002] (rev 07)
05:00.1 Input device controller [0980]: Creative Labs SB Live! Game Port [1102:7002] (rev 07)

I know about KXproject for Emu10k1, so I installed kxaudiodriver
I have verified, that my vendor ID and device ID is in kext .plist file, but still can't see output in system > audio

FYI:
- I don't use DSDT
- I'm booting from SW RAID
- my motherboard is Asrock 4Core-1600GLAN, CPU Q8300, GPU Ati HD4670

Update:

with this DSDT> http://dl.dropbox.com/u/7883291/dsdt/DSDT.aml.patched_by_artur.zip SBLive is working, I can hear all 5+1 channels, but. S/PDIF (aka Digital Out) don't work and my resolution screen is 1024x768.

I'm new it hackintosh, can someone pls. help me ?
