When you open Hackintool and navigate to the PCIe tab, what does it show under IOReg Name for your ATI HD 5450?
I used a couple of these many years ago and created an SSDT which made them work in macOS. The IOReg of my card(s) was _SB.PCI0.POP2.GFX0. If you tell or show me (screenshot) of the IOReg Name for your card I can edit the SSDT to hopefully work with your HD 5450.

Other than injecting WhateverGreen.kext I didn't need any other fixes or patches to get the card working.
