currently i'm using clover r4128.

The kexts in EFI/CLOVER/kexts/10.12 are of very old versions and config.plist/Graphics is incomplete. Also, you need Csr=0x67. Try this Clover folder. Your problem is not a kernel panic btw.

Assuming you're booting the installer from USB, your verbose output implies a USB problem.
