my system is
10.10.5 Yosemite
i7 4790
Z97X UD5H
GTX 690
16GB ram 4*4 Ram

System Defintion as Mac Pro 3.1

no issue for few month, but suddenly this issue appeared.

FYI black screen can be caused if your injecting. Be sure that your not injecting. Clover (Inject nvidia False) Chameleon GraphicsEnabler=No.

i did not use any CLOVER , just followed the normal guide using Multibeast

note : using NVIDIA Web Driver : 346.02.03f05 ( up to date) as of now

Check org.chameleon.plist for GraphicsEnabler=Yes. Also check BIOS. Sometimes things can change.

Why do you have IGP enabled when your using dedicated Graphics?
