Mac-F221BEC8 is a MacPro5,1 which does not meet the minimum requirements for the OS version that you are attempting to install. Change your SMBIOS or change your OS.

The usual cause of that issue is an old version of Clover.

Try adding npci=0x3000 to your boot arguments.
