Your copy of Clover is very old (v3113). Same goes for the kexts you have in Clover/kexts.

Also, GenericUSBXHCI.kext is not required.

No idea what you're referring to with "installation guide page".

After changing clover to latest version and trying to boot into installer..it stuck again on a apple logo..by verbos mod it show this eror:root device uuid is a5dd4bba-c1b7-363c-aaac-434e8ce5b47b

Boot verbose, show photo. Post EFI/Clover folder as ZIP (omit 'themes' folder).
