I made both versions for you (But ideally yours should have been identical).

Any ideas why the 4331cd card would not work on a z77 ds3h f9 bios?

tried quite a bit to get handoff to work on this. Its using 10.10.2, smbios imac 13,2 (I believe card was made for)
last thing tried was the whitelist dsdt edit tho didnt think was needed for that smbios. All that acheived was error in logging into icloud: valid id but not an icloud account. Noticed network stack mentioned in ioregistry and tried enabling in bios too.

Through Clover or on-disk? Also what is the current country code your card is displaying?

Through clover, and the country code is US/FCC

If your card is US/FCC you should have 5ghz channels showing.
Please post a screenshot of the system profiler WiFi section, and mention your vendor / device id.

Look at the country showing under system profiler, "XT" or "US"

Try the XT-ROW replacement patch:

In AirportBrcm4360.kext:
Find: 58 54 00 5D 19 54 21 00 00
Replace: 58 54 00 8D A8 5E B6 00 09

Its noted in an earlier post in this thread.

Still no go.
I am going to list out every possible aspect that I have added/changed to make sure I did not miss anything.

1. I have BcrmPatchRAM.kext, FakePCIID.kext, FakePCIID_BCM94352Z_as_BCM94360CS2.kext installed in EFI/EFI/CLOVER/kexts/10.10

2. I have the following Kernel Hex Patches: Screenshot attached.

3. I rebuild kext cache everytime I make a change in Clover Configurator

4. I am running 10.10.2 using Clover, so S/L/E is untouched.

5. My WiFi and Bluetooth settings from System Report attached as screenshots.

6. If it helps, my system is based on an GA-H97 WiFi motherboard with a Core i3 3.6Ghz Dual Core processor and nVidia GTX 760 GPU.

7. My Bluetooth/WiFi card is the Half Mini PCIe Azurewave CE123H.

8. Vendor ID and Device ID from DPCI Manager is attached.

Let me see when I get home what the problem might be.
