GTX 640 - Max 2 Displays?

Hey everyone - my setup:

Motherboard: GA Z77X-UD5H
CPU: Intel i7 3770K 3.5 GHz
GPU: GeForce GTX 640

The GPU has 3 outputs - two DVI, and one HDMI. I have HDMI audio enabled by making my DSDT and following the guide in the forums for HDMI audio. It works great.

However, for whatever reason, I can't get the graphics card to use all 3 outputs at one time. Trying to do 2 1920x1080 DVI monitors, and one HDMI TV at 1920x1080.

Any ideas for troubleshooting/diagnosing?

GT 640 supports two displays max. GT 650 and higher supports four displays.
