GPU & RAM Issues (OS X 10.11.6)

Good afternoon,

I've build an hackintosh based on the following components:
Gigabyte GA-Z97-HD3
Intel(R) Core(TM) i5-4570 CPU @ 3.20GHz
AMD Radeon HD 6850
3x 4GB DDR3 RAM @ 1333 MHz

and everything (audio, ethernet et cetera) is working fine except my graphics card and the 4. memory slot.

To the ram Issue:
With only 3 memory slots in use my computer is starting fine but when I add another RAM (1333 MHz 4gb) I can't even get into the bios/bios screen nor do my monitors work.

To the graphics card issue:
OS X recognises my GPU properly but when I use my hackintosh sometimes fragments are appearing. When I wake up the system from sleep mode it happens sometimes that the second monitor does not wake up properly just showing fragments all over the screen (which can be fixed when going back to sleep waking up the system again some minutes later)

I hope some of you can help me with this kind of problems.

Try swapping the memory around to see if it's the slot or the stick of memory that's bad.

I don't have anything to offer on the GPU problem.

Thx, I swapped the memory around and found out that one stick was broken, fixed it by buying a new 4gb DDR3 stick.
