Weird as it may sound "IOConsoleusers: gIOScreenLockState 3, hs 0, bs 0, now 0, sm 0x0" error was not related to any graphic card in my case. It was the hard drive. I was able to install fresh on a new drive and also migrate the data from the old drive. I formatted the old drive and tried to reinstall on it, but ran into the same issue. Unfortunately running diagnostics on the old drive did not show any issues, so it is confusing.
