Disable Fusion drive - how Make it back 2 separate drives ?

Hello
I've created a fusion drive with my 1 SSD and 1 HDD with a manual I've found in this great forum.
i would like to know how can i reverse the process and get back a single 1 SSD and 1 HDD.
I've a Yosemite OSX installed on this fusion drive so I'm guessing i would need to use unibeast USB to get into terminal and Disk utility but i don't know what orders to write in terminal to make them separate and than format them.

any help will be appreciate

1a. If your Fusion Drive is the startup volume, then you will need to boot with the Mountain Lion 10.8.2 Installer USB or another OS X 10.8.2 bootable drive. Once in Terminal type "diskutil coreStorage revert <UUID>"

$ diskutil coreStorage revert <UUID>

*** You will have you use your own (LVG) Logical Volume Group's UUID to complete step 1a above.
