Set your iGPU(HD4000) to primary with 64MB memory size in BIOS.

as imate says the ram size matters.

Also suggest you use these boot option commands at the startup when you boot from the USB "-v PCIRootUID=0"

Also use a USB 2 port not a 3.0 Port ( should also use a 2.0 USB drive)
