BCM4352 wifi + Bt don't work

Hi. I have a bit problem. My laptop is a Samsung rc520 with Sandy Bridge i5-2410M; 8GB; HD 3000. I have change my wifi card and I have installed minipci-express 802.11AC BT 867Mbps BCM4352 BCM94352HMB Dell DW1550 Mackintosh. I have insert the kext of the picture attached for wifi e bluetooth(also BrcmBluetoothPatchRam, etc) but Bluetooth doesn't exist and wifi don't see all wifi (probably 5Ghz don't work). I have used a guide:

    1. 10.10 and newer, 5GHz

        1. Select patch:

            1. Handoff/BCM94352/US-FCC
          2. Patch: 3

    2. Clover/kext patch
      1. Download (View Raw)
        1. 10.10 and newer, config-bcm94352-110.plist.zip
      2. Paste patch to config.plist/KernelAndKextPatches/KextsToPatch
        1. 5GHz (select one)
          1. BCM94352-5GHz-US-FCC-dv

with my old wifi card, bluetooth worked after I put USBInjextAll kext. Now with my new wifi card bluetooth don't work. I don't think is a usb problem.
Then who I enable 5 Ghz? I don't know. I'm very disperate.

I tried a guide for 5Ghz wifi but don't work for me. Maybe I have to change the code for my country ITALY and not US/Fcc but I don't know. You can help me??
Then I tried every kext brcm for Bt but don't appear nothing in USB info. Who I can do?
With last wifi card, bt work with UsbAllInject kext. Now no. Help me please. Give me a hand cause I don't know to do. I don't speak a good english, so sorry for my bad english

Hi. I've tried every patch included in post #1 for 5Ghz Patch. I have also change country code but I don't now because my wifi work only 2,4 Ghz. Now, what I do?

I find the 5ghz patch is not needed on 10.12.x (as per FAQ).
No "Problem Reporting" files attached.
 
So. I have a laptop with mac os el capitan. My pc is a samsung rc 520 ( Intel i5 Sandy Bridge series 6, i2410m hm65, 8gb ram, intel hd300) with this motherboard http://www.ebay.com/itm/141394408107. I have installed this wifi card minipci-express 802.11AC BT 867Mbps BCM4352 BCM94352HMB Dell DW1550 Mackintosh. I'm using clover for the installation.
After installation of wifi card work only 2,4 ghz and not Bt.
For 5 ghz i tried all step, patches by this guide https://www.tonymacx86.com/threads/guide-airport-pcie-half-mini-v2.104850/
But 5ghz don't work.
For Bt I used renamed EHC in EH0 and I have installed a usbinjectall.kext but don't appears.

I attach the file of problem reporting and same photo about old e new card and video about verbose mode of my pc.
I hope you can help me.
